Adult Day Services Facilitator - Portal, inc - Grafton, WI

Portal, inc is a human service agency supporting adults with developmental/intellectual disabilities. We currently have openings within our Adult Day Service Program.

Assist with organizing and providing community- and center-based educational, recreational and leisure activities/events for adults with developmental disabilities.

Core Responsibilities:
– Coordinate and implement events and activities in a caring and safe manner.
– Assist with planning and preparation of such events and activities.
– Complete all required records, reports and documentation
– Provide transportation (no special drivers license required) to/from events and activities.

Key Requirements/Skills:
– Excellent interpersonal and communication skills
– Valid WI drivers license with clear record
– Prior experience in direct care-giver/support role preferred.
